They visited Sage Center and reviewed housing options and met one-on-one with an eldercare counselor who served as both a listener and advisor. The counselor discovered Phil was struggling with all of Martha's care. A plan of care was developed with support services that helped them stay at home. According to Phil “the counseling at Sage Center is a treasured lifeline for anyone dealing with life changes.” Services A place to start for answers, resources, and services for aging needs (perfect for older adults, caregivers, and Baby Boomers) Access to Geriatric Care Management and other aging services An Expert Team to provide information for seniors and their families Resource materials, multi-media center, help with online applications, internet aging sites, and housing directories. Individual caregiver counseling with certified counselors Family caregiver mediation Caregiver peer support groups Grandparent Raising Grandchildren support groups Educational sessions like: “Powerful Tools for Caregivers, “Preparing for the Future” and “Getting your Affairs in Order” Wellness seminars Volunteer Opportunities at Alternatives for the Older Adult Gail’s household and job suffered when she had to step up the care for her father. As his needs became more demanding, she became overwhelmed. Gail went to Sage Center and met with a Caregiver Counselor. She learned quickly that she didn’t need to face all her difficulties alone.
