-Care Managers provide free in-home evaluations of an older person's needs.

-Care Managers provide Choices for Care hospital visits regarding options before nursing home placement.


Bridges Community Center

-Alternatives’ senior center offers social, educational, and recreational activities for seniors and caregivers.

-Bridges’ service area is the lower two-thirds of LaSalle County, though all seniors and caregivers are welcome.

-The center is the hub for information, assistance, and outreach, “bridging” seniors and caregivers to important and essential services

-Alternatives offers an access point to arrange and coordinate care for older adult, aged 60 and above, with health problems; not limited to publicly funded services

-Screened and approved volunteers meet regularly and meet one-on-one with clients to help organize and pay bills.

-Bill payer and representative payee services can be coordinated.

(Telephone Reassurance)

-Ring for Care volunteers provide safety and socialization telephone calls to isolated, homebound elders.
